ADVERTISEMENT FOR BID 2012 STREET MAINTENANCE PROJECTS NORTH OGDEN CITY CORPORATION RECEIPT OF BIDS: Sealed bids will be received at the office of the City Recorder, North Ogden City Office, 505 East 2600 North, until 10:00 am on April 25, 2012 , for the 2012 STREET MAINTENANCE PROJECTS. DESCRIPTION OF WORK: The work consists of: 19 tons of crack seal, 81,310 s.y. of slurry seal work, 24,523 s.y. of Light Weight Aggregate (Utelite) type "C " Chip & seal with fog seal, roadway patch repairs, level course, 767 s.y. of asphalt edge milling, 30,283 s.y. of overlay fabric & 3,125 tons of 1 " asphalt overlays, lowering manholes prior to milling, raising manholes, water valves, and monuments to grade and placing a concrete collar around them, removing and replacing 205 s.f. of 5 ' wide concrete waterway, 188 l.f. of 30 " curb & gutter, 415 s.f. of 6 " thick driveway approach, and 1,182 s.y. of 28 " deep asphalt and sub-grade rebuild. LOCATION OF THE WORK: The work is located on various streets throughout North Ogden City. COMPLETION OF THE WORK: All work covered by the contract documents shall be completed by: Sch. "A " - Crack Seal by July 15, 2012, Sch. "B " - Slurry Seal by August 31, 2012 , Sch. "C " - Chip & Seal By August 15, 2012, and Sch. "D " Asphalt work by September 15, 2012. Work can not begin until July 1, 2012! OPENING OF BIDS: The bids will be publicly opened at 10::00 a.m. on April 25, 2012 at the above mentioned office of North Ogden City, 505 East 2600 North. OBTAINING CONTRACT DOCUMENTS: Contract Documents, including Drawings and Technical Specifications, may be obtained at the office of the Engineer; Jones and Associates, 1716 East 5600 South, South Ogden, Utah 84403; upon application and payment of a $30.00 non-refundable fee. Plans will be available after Friday, April 6, 2012. BID SECURITY: Each bid shall be accompanied by a certified check, cashier 's check or bid bond in the amount of five percent (5%) of the total bid price payable to NORTH OGDEN CITY CORPORATION as a guarantee that the bidder, if his bid is accepted, will promptly execute the Contract, secure payment of worker 's compensation insurance and furnish a satisfactory, faithful performance bond in the amount of one hundred percent (100%) of the contract price and a payment bond in the amount of one hundred (100%) of the contract price. WAITING PERIOD BEFORE AWARD: A waiting period of 30 calendar days from the date of opening of bids to award of contract may be required. Bidder 's shall assume full responsibility for and shall guarantee the bid price during this period and make certain the time period is stated in and does not restrict the proposal guarantee. AWARDING PROCESS: The contract will be awarded using the "Choosing by Advantages Decision Making System ". In addition to the bid amount, each proposal will be submitted with documentation to support the following factors: (1) Number of years experience as a general contractor (2) List of projects of similar size and scope completed within the past 5-years (3) List of key personnel with their qualifications and experience assigned to this project. (4) List of sub-contractors. (5) Is contractor 's base of operation located in North Ogden City? (6) List of projects of similar size and scope completed in North Ogden City within the last 5 years. This documentation will be used in the choosing by advantages awarding process. PROJECT ADMINISTRATION: All questions relative to this project prior to the opening of bids, shall be directed to the Project Engineer. It shall be understood, however, that no specification interpretations will be made by telephone, nor will any "or equal " products be considered for approval prior to award of contract.
