ADVERTISEMENT FOR BID 2013 STREET MAINTENANCE PROJECTS NORTH OGDEN CITY CORPORATION RECEIPT OF BIDS: Sealed bids will be received at the office of the City Recorder, North Ogden City Office, 505 East 2600 North, until 10:00 am on June 19, 2013, for the 2013 STREET MAINTENANCE PROJECTS. DESCRIPTION OF WORK: The work consists of: Placing 85 tons of level course, 14,124 s.y. of geotextile overlay fabric, and 1,458 tons of 1 " overlay asphalt in the Ben Lomond Estates Subdivision area, raising to grade and placing a concrete collar on manholes and water valve boxes. Also consists of placing 60 tons of level course on Mountain road and placing 50,918 s.y. of light weight aggregate Chip & Seal with a GSB-88 Fog Seal over Mountain Road & Fruitland Drive. Other work consists of 1,774 s.y. of asphalt rebuild on 400 East which includes excavating down 28 ", place separation fabric, install 18 " thickness of 3 " minus granular borrow, 6 " thickness of roadbase, and 4 " thickness of asphalt in 2 lifts. In other areas of the City, there will be a total of 202 l.f. of curb & gutter remove and replace, 412 s.f. of 4 " flatwork remove & replace, 4 truncated dome panel placements for handicap ramps, 1,008 s.f. of 5 ' wide reinforced concrete waterway replacement, and 1,130 s.y of asphalt patching which includes excavating down 15 ", place separation fabric, install 12 " of roadbase and 3 " of asphalt. LOCATION OF THE WORK: The work is located on various streets throughout North Ogden City as shown on the project location map. COMPLETION OF THE WORK: All work covered by the contract documents shall be completed by: Sch. "A " - Asphalt & Concrete work by September 15, 2013 and Sch. "B " - Chip & Seal By August 15, 2013. Work can not begin until July 1, 2013 ! OPENING OF BIDS: The bids will be publicly opened at 10::00 a.m. on June 19, 2013 at the above mentioned office of North Ogden City, 505 East 2600 North. OBTAINING CONTRACT DOCUMENTS: Contract Documents, including Drawings and Technical Specifications, may be obtained at the office of the Engineer; Jones and Associates, 1716 East 5600 South, South Ogden, Utah 84403; upon application and payment of a $30.00 non-refundable fee. Plans will be available Monday, June 3, 2013. BID SECURITY: Each bid shall be accompanied by a certified check, cashier 's check or bid bond in the amount of five percent (5%) of the total bid price payable to NORTH OGDEN CITY CORPORATION as a guarantee that the bidder, if his bid is accepted, will promptly execute the Contract, secure payment of worker 's compensation insurance and furnish a satisfactory, faithful performance bond in the amount of one hundred percent (100%) of the contract price and a payment bond in the amount of one hundred (100%) of the contract price. WAITING PERIOD BEFORE AWARD: A waiting period of 30 calendar days from the date of opening of bids to award of contract may be required. Bidder 's shall assume full responsibility for and shall guarantee the bid price during this period and make certain the time period is stated in and does not restrict the proposal guarantee. AWARDING PROCESS: The contract will be awarded using the "Choosing by Advantages Decision Making System ". In addition to the bid amount, each proposal will be submitted with documentation to support the following factors: (1) Number of years experience as a general contractor (2) List of projects of similar size and scope completed within the past 5-years (3) List of key personnel with their qualifications and experience assigned to this project. (4) List of sub-contractors. (5) Is contractor 's base of operation located in North Ogden City? (6) List of projects of similar size and scope completed in North Ogden City within the last 5 years. This documentation will be used in the choosing by advantages awarding process. PROJECT ADMINISTRATION: All questions relative to this project prior to the opening of bids, shall be directed to the Project Engineer. It shall be understood, however, that no specification interpretations will be made by telephone, nor will any "or equal " products be considered for approval prior to award of contract.
