ADVERTISEMENT FOR BID 2012 C.D.B.G. - GREEN ACRES STREET REPAIR PROJECT PHASE 2 NORTH OGDEN CITY CORPORATION RECEIPT OF BIDS: Sealed bids will be received at the office of the NORTH OGDEN CITY RECORDER, 505 East 2600 North, North Ogden, Utah, until 10:00 a.m. on April 9, 2013, for the 2012 C.D.B.G. - GREEN ACRES STREET REPAIR PROJECT PHASE 2. At which time, bids will be opened publicly. DESCRIPTION OF WORK: The work consists of edge milling 6 ' wide 4,415 s.y., pre-lower 7 ea. water valves and 4 ea. manholes, raise to grade and collar 19 ea. water valves and 16 ea. manholes, 92 s.y. asphalt patching (12 " UTBC, 3 " AC), 90 ton level course, 14,414 s.y. Geotextile fabric with tack coat, 1,362 tons of 1 " Bituminous surface course overlay. LOCATION OF THE WORK: The work is located on 1950 North - 450 East to 500 East, 1850 North - 450 East to 500 East, 1800 North - 450 East to 475 East, 475 East - 1750 North to 1800 North, 1750 North - 450 East to 600 East, and 1725 North south of 1750 South in North Ogden City, Utah. COMPLETION OF THE WORK: All work covered by the contract documents shall be completed by August 31, 2013. Work can not begin until July 1, 2013 ! OBTAINING CONTRACT DOCUMENTS: Contract Documents, including Drawings and Technical Specifications may be obtained at the offices of Jones & Associates Consulting Engineers, 1716 E. 5600 S., Ogden, Utah, upon application and payment of a $ 30.00 fee. Plans will be available after Wednesday, March 27, 2013. BID SECURITY: Each bid shall be accompanied by a certified check, cashier 's check or bid bond in the amount of five percent (5%) of the total bid price payable to NORTH OGDEN CITY CORPORATION as a guarantee that the bidder, if his bid is accepted, will promptly execute the Contract, secure payment of worker 's compensation insurance and furnish a satisfactory, faithful performance bond in the amount of one hundred percent (100%) of the contract price and a payment bond in the amount of one hundred (100%) of the contract price. WAITING PERIOD BEFORE AWARD: A waiting period of 30 calendar days from the date of opening of bids to award of contract may be required. Bidder 's shall assume full responsibility for and shall guarantee the bid price during this period and make certain the time period is stated in and does not restrict the proposal guarantee. AWARDING PROCESS: The contract will be awarded using the "Choosing by Advantages Decision Making System ". In addition to the bid amount, each proposal will be submitted with documentation to support the following factors: (1) Number of years experience as a general contractor (2) List of projects of similar size and scope completed within the past 5-years (3) List of key personnel with their qualifications and experience assigned to this project. (4) List of sub-contractors. (5) Is contractor 's base of operation located in North Ogden City? (6) List of projects of similar size and scope completed in North Ogden City within the last 5 years. This documentation will be used in the choosing by advantages awarding process. PROJECT ADMINISTRATION: All questions relative to this project prior to the opening of bids, shall be directed to the Project Engineer. It shall be understood, however, that no specification interpretations will be made by telephone, nor will any "or equal " products be considered for approval prior to award of contract. OWNER 'S RIGHTS RESERVED: The Owner reserves the right to reject any or all bids, to waive an informality in a bid, and to make awards in the interest of the Owner. PROJECT FUNDING AND AWARD: This project is being funded with a Federal Community Development Block Grant and all appropriate federal labor laws will apply, including compliance with Section 3 (attempts to hire local and minority individuals) and payment of "Davis Bacon " wages.
