REQUEST FOR PROPOSAL FOR ARCHITECTURAL SERVICES FOR THE UINTAH CITY FIRE STATION REMODEL/UPGRADE PROJECT UINTAH CITY CORPORATION RECEIPT OF PROPOSALS: Proposals must be delivered to the office of the project manager Lee Wohlgemuth, 2191 East 6550 South, Uintah, Utah, 84405 on or before 5 PM on Monday, December 1, 2012, for the Fire Station Remodel/Upgrade in Uintah City. DESCRIPTION OF WORK: Proposals shall include architectural services as outlined in the detailed RFP documents available at City Hall, located at 2191 East 6550 South, Uintah, Utah, 84405 or online at http://www.jonescivil.com/uintah.htm the office of the city engineer. SELECTION OF ARCHITECT: Selection of the successful Architect/Firm will be based upon a matrix giving a predetermined value to each of the submissions required in the detailed RFP. Within 30 days up to three Architects/Firms will be selected to participate in an interview process. Within two weeks following the interviews the successful Architect/Firm will be notified. PROJECT ADMINISTRATION: All questions relative to this project prior to the opening of the bids shall be directed in writing to the project manager Lee Wohlgemuth, the sole authorized contact person for this project. It shall be understood that no alterations to the requirements of the proposal will be made by telephone. Any clarifications or alterations to the RFP will be made in writing. SPONSOR 'S RIGHTS RESERVED: The Sponsor reserves the right to reject any or all proposals, to waive any informality in a proposal, and to make awards in the interest of the Sponsor. CONTRACTING PROCESS: The successful Architect/Firm will be required to enter into a written agreement with the Sponsor in a form acceptable to the Sponsor. C.D.B.G. NOTICE: This project is funded through a Department of Housing and Urban Development (HUD) Community Development Block Grant (C.D.B.G.). The Architect/Firm will be required to comply with all federal labor standards and attendant laws, including payment of the most current Davis-Bacon wages and compliance with Section 3 to provide employment opportunities for lower income persons.
